Er, pretend that ')' is '()'.  For me it's usually when I'm closing a quote, but I'm not the only real person and all.

Also, less keyboard typo, more brain typo.

Yeah, I didn't think to make the distinction until you'd commented about Dvorak.  For keyboard typo, I'm more likely to fail to hit the Shift key.

Just for some reason, my brain really doesn't care about the difference between closing a parenthetical and closing a quote, and figures either closing thing'll do.

Do you mean switch as in type one when you want to type the other? Or switch their order?

I switch all sorts of delimiters: ), ', ", *, ] ... my brain thinks "endy thing goes here" and chooses one at random, and my fingers obey before the proofreading part of my brain catches up and says "Hey, your delimiters don't match!"

While I do have certain typical typos (which have started showing up in my handwriting >.<) that's not one of them. Mine generally involve accidentally switching off hands overly much (resulting in "tlak" and "wlak", for example).

I only do it in where I use " instead of ). But I do it nearly every time.
